Login ID: *Your Login ID is the PID found in your Order Form (under "Patient Details").
Password: *Your password is your Last Name (in CAPITAL letters).
*If you have name suffixes (JR, II), it will be included in the Last Name.
For example, FERDINAND JOSE JR = JOSEJR

Login ID: *This is the unique ID given to you by Hi-Precision Diagnostics.
Password: *For first time users, this is the Password given to you by Hi-Precision Diagnostics.
